Argh!  I had searched all through the documentation and googled the
software's home page, found nothing, and came to the conclusion that
I'd have to hack the tex4ht sourcecode in order to inline footnotes
(and, according to another article that I came across, hacking the
source sounds really nasty).  Turns out that the documentation for
tex4ht is just really crummy.

I feel like a fool.

For future google reference, the syntax for inlining footnotes with
tex4ht is `htlatex foo.tex "html,fn-in"` (or some variant thereof).

The results aren't ideal but I suspect that it's a close as an
automated procedure will get (and more than sufficient for many
purposes).  A Word macro is probably sufficient to get things perfect.
