Do you assign a unique key to each customer?  Are they customer numbers?

MySQL (old, 3.23, but it works well, debating whether to upgrade to 4.x) and I 
have indexed what I can, but there are no columns on which to base a unique 
key. The data is all names,dob,dates,etc. and there can/are multiple records 
for the same individuals with differing information.
